Failed to format this volume to the level-2 storage!

I've received the failed to format error as well and have confirmed the SSD drive to be MBR. I've attached in image with the error and the disk management in the background.
Server specs:
Windows Server 2003 R2 x64 SP2
Pentium D 830
4 GB DDR2 667
Supermicro PDSM4+
OCZ Vertex 3 90 GB
I was previously using this drive as L2 cache with the latest version of FancyCache.

Pending an official response, does the SSD format normally and does partitioning it (and using PrimoCache to format one partition only) make any difference?

Yes, I can format the SSD with the Windows Disk Management. PrimoCache returns the format error on either a pre-formatted partition or unformatted partition.
I was using this SSD in FancyCache as an L2 cache with no issues.

And perhaps drive A:/B: is not allowed. Could you try another drive letter?

Yes, that was the first thing I checked before posting. You can see that in the screenshot I attached to my first post.
Changing drive letters resulted in the same error.

So you're sure that it is not a GPT-based disk? From the screenshot you uploaded, I can only confirm that it is a basic disk, not a dynamic disk, but I'm not sure if it is a MBR-based disk.
Otherwise, I'm afraid that we have to wait the next version which will log detailed error information to find where the problem is.

It seems I made a mistake. The option to change to and from MBR and GPT isn't available unless I delete all the partitions. After removing all the partitions I gained the option to change it.
What I find unusual about this, though, is that I was using that drive with FancyCache previously and didn't purposely change it to GPT. Is that a new requirement for the newest version that wasn't in the previous?

Previously FancyCache doesn't need a dedicated partition for level-2 storage. So now such requirements at all. However for better performance and overcoming some bugs in FancyCache, we use a new way in PrimoCache.
